What is MyChart?

MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to manage and receive information about your health securely via the Internet. With MyChart, you can use your online MyChart account to:
*Request medical appointments.
*View your health summary from the MyChart electronic health record.
*View test results.
*Request prescription renewals.
*Access trusted health information resources.
*Communicate electronically and securely with your medical care team.

Is there a fee to use MyChart?

MyChart is a free service offered to our patients.

Do I need a different username and password for OhioHealth MyChart and MyChart Powered by OhioHealth?

No. Your MyChart username and password is the same for OhioHealth MyChart and MyChart Powered by OhioHealth.

What is the difference between OhioHealth MyChart and MyChart Powered by OhioHealth?

The MyChart content and functionality are the same for both OhioHealth MyChart and MyChart Powered by OhioHealth. The MyChart screens may have a unique look.

How do I sign up for MyChart?

Patients who wish to participate will be issued a MyChart activation code during their outpatient doctor office visit. This code will enable you to log in and create your own username and password. If you were not issued an activation code, you may select 'Sign Up Now' then 'SIGN UP ONLINE' or ask to sign up during your next office visit.

Who do I contact if I have further questions about using MyChart?

When can I see my test results in MyChart?

Your test results are released to your MyChart account generally within 5 days. Note that not all test results may be available through MyChart.

Are all test results shared electronically via MyChart?

No. While most test results are shared electronically some results are not available through MyChart.

What if I want access to my full medical record?

MyChart provides patients with access to pertinent portions of their medical records. If you would like a copy of your medical record, you may contact your physician's office or the location where you received your medical care.

If I believe that some of my health information on MyChart is not correct, what should I do?

Your MyChart information comes directly from your electronic medical record at your doctor's office. You may ask your doctor to correct any inaccurate information or obtain an amendment request form at your next office visit. Your health information is reviewed and updated in your electronic medical record each visit. If I send a message to my doctor, when can I expect a reply?

You will generally receive an answer within 1-3 business days. It is important that you do not use MyChart for emergencies or issues requiring urgent attention. Please call 911 if you have an emergency or urgent medical question. If you are unsure of the urgency of your issue, please call your doctor's office to speak to the doctor on call.

What happens to information I send to my providers through MyChart?

Information you send to your health care professional through MyChart may be added to your medical record upon review. Once added to your medical record, that information may be used or disclosed with your medical record as described in the Notice of Privacy Practices (for OhioHealth patients) or the Notice of Privacy Practices where you received care..

Can I view a family member's health record in MyChart?

Yes. To gain access to another MyChart user's (age 14 and older) account, ask them to send you an invite to view their MyChart account. They would log into MyChart using MyChart.OhioHealth.com, select ‘Share My Record’ under the Health menu, select 'Friends and family access' and 'Invite Someone'. You will receive an email with a link to accept the invitation and log in to your MyChart to complete the invitation. To gain access to a child’s (under the age of 14) health record, complete the form, MyChart Authorization for Proxy Access , to request proxy access. Certain criteria must be met to be approved as a patient's proxy.

Can I give other people access to my health record in MyChart?

Yes. You can invite other MyChart users (age 14 and older) to view your account. Login to MyChart, select 'Share My Record' under the Health Menu, select 'Friends and family access' and 'Invite Someone'. The person you are inviting will receive an email with a link to accept your invitation by logging in to their own MyChart to complete the invitation. If the person you wish to invite does not have a MyChart account, please complete the form MyChart Authorization for Proxy Access , to request proxy access.

Can my child have a MyChart account?

Children of any age can have a MyChart record that can be accessed by their parent/guardian's MyChart account. A minor 14 years of age and older may have his or her own MyChart account which he or she may control with a unique username and password. Under State and Federal law, there are certain types of medical information that a parent/legal guardian of a minor patient may not view without consent of the minor patient. A minor who allows the parent/guardian to access the minor's account is permitting the parent/guardian access to information that may be confidential under State and Federal law.

Can I ask questions regarding a family member from my MyChart account?

No. Because your communications in MyChart are recorded in your personal medical record, this information would not appear in the correct medical record and could potentially jeopardize medical care. If you are participating in a family member’s medical care and would like to communicate with that family member’s providers, see the question above regarding viewing a family member's health record in MyChart.

Can my spouse and I share one MyChart account?

No, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own MyChart account.

I forgot my password. What should I do?

You may click the "Forgot password" link on the sign-in page to reset your password online. You may also contact our MyChart Patient Support Line at 1-614-533-MyChart (6924) or 1-844-OHMyChart(646-9242) to request a new, secure password.

Can you send me a new activation code as I have lost it, let it expire or did not receive it?

Contact us at MyChartSupport@OhioHealth.com and after we verify your information, a new code will be sent via U.S. Postal Mail.

Where can I update my personal information (e.g., home address, e-mail or change my password)?

Log into MyChart and under Profile, select Personal Information to change your contact information or Security Settings to change your password.

How do you protect the information in MyChart?

We afford the same degree of confidentiality to medical information stored on MyChart as is given to medical information stored by OhioHealth in any other medium. OhioHealth and your health care provider are committed to protecting the confidentiality of your (or in the case of Proxy, the patient’s) medical information. OhioHealth uses reasonable care to maintain your identifiable and personal medical information in MyChart privately and securely. Access to MyChart is controlled through individual activation codes, unique usernames and passwords. Each patient controls his or her own password, and your MyChart account cannot be accessed without that password. Further, MyChart uses industry-standard 128-bit Secure Socket Layer (SSL) encryption technology, with no caching, to automatically encrypt your MyChart session.

What is your Privacy Policy?

MyChart is owned and operated by OhioHealth. The information contained in your MyChart account will be governed by the Notice of Privacy Practices for OhioHealth patients (or, for MyChart powered by OhioHealth users, by the Notice of Privacy Practices for your healthcare provider).

What do I need to use MyChart?

You need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer).

My activation code does not work, what should I do?

For your security, your activation code expires after 60 days and is no longer valid after the first time you use it. If you still have problems, email us at MyChartSupport@OhioHealth.com or you can call our MyChart Patient Support Line at 1-614-533-MyChart (6924) or 1-844-OHMyChart(646-9242).

What are my obligations when using MyChart?

You must agree to the MyChart Terms and Conditions of Use and must use MyChart in accordance with the Terms and Conditions of Use.

Can I import health information from my health record into a third-party application?

Yes, you can share information with a third-party application if you have an OhioHealth MyChart account. Your MyChart username and password would be the authentication used by the application to access and import your health information.

Is there an app you recommend that would work with the patient portal?

No. OhioHealth does not endorse any third-party applications or vendors. The app(s) that our system has available are on Epic’s App Orchard

How do I connect my third-party app if I do not see it listed as an available option?

The vendor of that application should supply instructions on how to connect their application. Applications will contain a list of facilities. If OhioHealth is not on the list, you will need to contact the vendor and ask them to connect to OhioHealth.

I am a third-party application developer; how can I connect to OhioHealth FHIR API to enable this functionality for your patients that use our app?
